Job Summary:

We are seeking a compassionate and detail-oriented Triage Nurse to assess and prioritize patient care based on medical urgency. The ideal candidate will have strong clinical judgment, excellent communication skills, and the ability to work in a fast-paced environment. This role involves evaluating patient symptoms, determining the level of care needed, and providing appropriate guidance, whether in-person, over the phone, or via telehealth services.

Key Responsibilities:

• Assess patients’ symptoms and medical histories to determine the urgency of care.
• Prioritize and direct patients to the appropriate level of care (e.g., emergency care, urgent care, primary care, or self-care).
• Provide initial nursing interventions, including vital signs assessment and basic treatments.
• Educate patients on self-care and symptom management when appropriate.
• Communicate effectively with physicians, nurses, and other healthcare professionals.
• Document assessments, recommendations, and patient interactions accurately in electronic health records (EHR).
• Follow protocols and guidelines to ensure high-quality, efficient triage decisions.
• Support telehealth and phone triage services by evaluating patient concerns remotely.
• Maintain up-to-date knowledge of triage protocols, emergency procedures, and best practices.

Qualifications & Requirements:

• Education: Registered Nurse (RN) license required; BSN preferred.
• Minimum 3 years of nursing experience, preferably in emergency medicine, urgent care, or primary care settings
